Cataract Surgery in India: Costs, Lens Choices and What to Expect

Cataract surgery in India is among the most established operations in the country’s eye hospitals, and its price range is wide. A basic operation and a premium laser-assisted one can differ by more than ten times. This guide explains why, and when travelling for surgery actually makes financial sense.

What a cataract is

A cataract is a clouding of the eye’s natural lens. Light scatters instead of focusing cleanly, so vision turns hazy, contrast drops, glare increases and night driving gets harder. Prescription changes can be an early sign.

Glasses cannot make a clouded lens clear. Surgery removes it and replaces it with an artificial intraocular lens (IOL).

When surgery is considered

The question is not how “ripe” the cataract looks. It is whether your vision interferes with daily life, work or safety, and whether updated glasses can still cope. Occasionally there are medical reasons, such as a swollen lens raising eye pressure or a cataract blocking the view needed to treat the retina.

Cataract surgery techniques: phaco and SICS

Phacoemulsification (phaco): through a tiny incision, ultrasound breaks the cloudy lens into fragments that are suctioned out. A foldable IOL is then inserted into the capsular bag.

Manual small-incision cataract surgery (SICS): the lens is removed in one piece through a self-sealing tunnel. It is often chosen for very hard cataracts or where phaco equipment is limited.

Newer options include micro-incision (MICS) and femtosecond laser-assisted surgery (FLACS). More advanced does not automatically mean better for your eye. The surgeon weighs cataract density, eye anatomy, equipment and experience.

Choosing an intraocular lens

  • Monofocal: sharp at one distance, usually far. Reading glasses are still needed.
  • Toric: also corrects astigmatism.
  • Multifocal or EDOF: aim to reduce dependence on glasses across distances, with trade-offs such as glare or halos.

Do not pick a lens only because it promises glasses-free life. The health of your retina, optic nerve and cornea limits what any lens can deliver.

When another eye disease is present

Many cataract patients also have glaucoma, diabetic eye disease, macular degeneration or corneal problems. Removing a cloudy lens lets in more light, but it cannot undo damage to the retina or optic nerve. A swollen cataract can also close the drainage angle of the eye, and in some patients cataract and glaucoma surgery are planned together. Dense cataracts can hide the retina, so an ultrasound scan may be needed. If a retinal problem is found, it can change both the plan and your expected result.

Recovery, complications and “secondary cataract”

Recovery after small-incision surgery is usually quick, though early vision can be affected by corneal swelling, inflammation and any existing retinal disease.

Cataract surgery is highly successful, but it is still surgery inside the eye. Possible problems include posterior capsule tear, vitreous loss, infection (endophthalmitis), raised pressure, IOL displacement, cystoid macular oedema and, rarely, retinal detachment.

If vision turns cloudy months or years later, the natural lens has not grown back. The thin capsule behind the implant has become opaque, which is called posterior capsule opacification. A short laser treatment usually clears it.

Cataract surgery cost in India compared with other countries

Option in India (per eye)INRApprox. USD
Phaco with standard foldable lens₹15,000 – ₹60,000$155 – $625
Micro-incision (MICS) with premium lens₹45,000 – ₹1,50,000$470 – $1,560
Femtosecond laser-assisted (FLACS)₹70,000 – ₹2,00,000$730 – $2,080
Premium lens (toric, multifocal, EDOF) adds₹30,000 – ₹1,00,000$310 – $1,040

An Aravind Eye Care System paper in the Community Eye Health Journal put a private phaco with a locally made foldable lens at roughly USD 150–250, and more than USD 500 with an imported aspheric lens (CEH Journal). Check the publication date, as hospital prices have moved since.

CountryPublished figure
United StatesAbout $2,500 – $3,500 per eye standard; $4,000 – $6,500 with premium lens and laser (Medibuddy comparison)

Turkey, both eyesAbout $4,300 (same source)
Spain, both eyesAbout $7,300 (same source)

Is travelling worth it?

It depends on your insurance. A peer-reviewed comparison of cataract surgery abroad and in the US found that for one eye, domestic surgery was a low-cost option once flights and lodging were counted (PMC study). Its figures are a decade old, so use it for the logic, not the numbers.

The saving is strongest when you are uninsured or underinsured, need both eyes done, or want a premium lens that insurance will not fund. If your plan or national health service covers a standard lens, compare carefully before you book flights.

Questions to ask before surgery

  • How much of my vision loss is caused by the cataract?
  • Is my retina healthy? Do I have glaucoma or corneal disease?
  • Which technique and which lens do you recommend for my eye, and why?
  • What vision should I realistically expect?
  • Can both eyes be done on this trip, and how long between them?
  • Which symptoms after surgery need urgent review?
